Regular follow-up to keep diabetes under control: blood-sugar checks, education and complication prevention.

Living with diabetes takes regular attention: watching blood sugar, understanding the effect of meals and medication, and spotting signs of complications early. Between medical appointments, nursing follow-up at home helps you stay on track, especially after a new diagnosis, a change in treatment, or when getting around is difficult.

At each visit your nurse measures your blood sugar, goes over your recent results with you and checks that you are comfortable with your meter and your treatment. She also examines at-risk areas such as the feet, and answers your questions about food and daily life. If something concerns her, she recommends that you contact your doctor.
