With age, blood pressure needs closer monitoring, and swings can cause dizziness or falls. Travelling to a clinic for a single reading is often out of proportion. Follow-up at home allows regular readings, taken calmly, with no travel effort.

Your nurse measures blood pressure sitting, and standing if needed, records the values and compares them with earlier ones. She checks that medications are being taken as planned and stays alert to signs such as dizziness, fatigue or swelling. With the person’s consent she can share a summary with a relative, and she recommends a medical visit if the values concern her.
